Most Popular Medicare Supplement Plans 2027: What 13.8 Million Members Chose

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 29, 2026

Plan G is the most popular Medicare Supplement plan, with about 6.3 million members (46%) at the end of 2025, followed by Plan F at 4.2 million (30%) and Plan N at 1.5 million (10%). Those figures come from NAIC filings compiled by Mark Farrah Associates and reported by KFF in October 2026, and UnitedHealthcare is the most popular company, covering about 31% of all members.

Popularity is the one thing about Medicare Supplement insurance you can actually count. Every insurance company files its enrollment with state regulators each year, and those filings show which plan letters people buy and which companies they buy from. This page reports those counts instead of opinions.

If you want our recommendations by situation, see the best Medicare Supplement plans for 2027. If you want every benefit side by side, use the 2027 comparison chart. Here, the question is simpler: what did real people choose, and what does that tell you?

Medicare Supplement Enrollment by Plan Letter, End of 2025

Most popular Medicare Supplement plans by enrollment: Plan G 6.3 million, Plan F 4.2 million, Plan N 1.5 million members at end of 2025

Three letters hold almost nine out of every ten Medicare Supplement policies. The other seven standardized letters, plus older plans like E, H, I and J that are no longer sold, share what is left.

Plan Members (Dec. 2025) Share of all members
Plan G About 6.3 million 46%
Plan F About 4.2 million 30%
Plan N About 1.5 million 10%
All other plans combined About 1.8 million (our calculation) About 14% (our calculation)

Source: KFF, Key Facts About Medigap Enrollment and Premiums (Oct. 13, 2026), using NAIC data compiled by Mark Farrah Associates. The ‘all other’ row is our subtraction from the 13.8 million total.

Reading the numbers: These counts include people who bought their policy many years ago. Plan F’s 4.2 million members are mostly people who joined Medicare before 2020, because no one newer can buy it.

How Plan G Took the Top Spot From Plan F

Plan F was the clear leader for years. The change started when federal law (MACRA) stopped the sale of Plan F and Plan C to anyone newly eligible for Medicare on or after January 1, 2020. From that day on, nearly every new buyer who wanted the fullest coverage chose Plan G instead.

Year Plan F Plan G Source
2019 49% of members 22% of members AHIP
2022 39% of members 36% of members AHIP
2024 About 4.5 million About 5.7 million Mark Farrah Associates
2025 About 4.2 million About 6.3 million Mark Farrah Associates

AHIP figures are shares of enrollment; Mark Farrah figures are member counts from NAIC annual statements.

Our math on the last two rows: Plan G added roughly 600,000 members in a single year while Plan F lost about 300,000. Plan F cannot grow again, so the gap will keep widening in 2027. For the full story on the two plans, read Plan F vs Plan G.

The Most Popular Medicare Supplement Companies

The plan letter sets the benefits, but the company sets the price. A handful of insurers sell most policies. The top 10 companies together held more than 9.8 million members, or 71% of the market, at the end of 2025.

Rank Company (parent) Members, Dec. 2025 Market share
1 UnitedHealthcare (UnitedHealth Group) About 4.27 million 31%
2 Mutual of Omaha About 1.3 million 9.4%
3 HCSC (Blue Cross and Blue Shield in Illinois, Texas, Oklahoma, New Mexico and Montana) Over 1.2 million Not published
4 CVS Health (Aetna) About 953,000 Not published

UnitedHealthcare. The largest by far. Read our UnitedHealthcare Medicare Supplement review.

Mutual of Omaha. Second place nationally. See our Mutual of Omaha Medicare Supplement guide.

Blue Cross plans. HCSC alone covers five states. Other Blue plans sell in their own regions.

Where Medicare Supplement Coverage Is Most Common

Most popular Medicare Supplement plans by state: Iowa 75 percent, Hawaii 14 percent, 46 percent nationally

Popularity also depends a lot on where you live. KFF found that 46% of people with traditional Medicare had a Medicare Supplement policy in 2024, which is about 12.9 million of 27.8 million people. But the share swings widely from state to state.

Highest: Iowa. About 75% of people in traditional Medicare had a Medicare Supplement policy in 2025.

Lowest: Hawaii. Only about 14% did.

Eleven states at 60% or more. In those states, a Medicare Supplement plan is the normal way to fill Medicare’s gaps.

Under 65 is rare. Only 6% of people under 65 in traditional Medicare had a policy, compared with 47% of those 65 and older.

The under-65 gap is mostly about access. KFF counts 38 states that require insurers to offer at least one policy to people under 65 during an initial enrollment window, and prices for that group are often much higher. If you live in Texas, see our Texas Medicare Supplement guide for that state’s rules.

What Members Pay for the Popular Plans

KFF also averaged what policyholders actually paid in 2025. These are real premiums across all members, not quotes for a new 65-year-old.

Plan Average monthly premium, 2025 Notes
All plans $244 ($2,926 a year) Up 7% from $228 in 2024
Plan G $194 ($2,323 a year) From $164 in New Mexico to $257 in New York
Plan F $325 Older, closed group of members

Source: KFF, Oct. 13, 2026, using NAIC data compiled by Mark Farrah Associates. KFF did not publish a Plan N average.

Why Plan F averages more: KFF points to two reasons. Plan F also pays the Part B deductible, and its members are an older group because no new enrollees can join. That is why a bigger plan letter is not always a cheaper one.

What the Popularity Numbers Mean for a 2027 Buyer

1

Treat Plan G as the default, not the answer. It is the most popular letter partly because Plan F closed. Check whether its premium in your ZIP code is worth it versus Plan N.

2

Look at Plan N if you rarely see a doctor. One in ten members chose it to save on premiums. Our Plan G vs Plan N 2027 guide shows the break-even math.

3

Do not pick a company only because it is big. In New Jersey’s April 2026 state chart, Plan G for a 65-year-old woman ran from $147.88 a month (Washington National) to $260.98 (Aetna), with UnitedHealthcare at $167.13. Same benefits, very different prices.

4

Buy in your open enrollment window. Your six-month Medigap Open Enrollment Period is when no company can turn you down for health reasons.

Bottom Line

Plan G is the most popular Medicare Supplement plan by a wide margin, and it gained about 600,000 members in 2025 alone while Plan F shrank. UnitedHealthcare and Mutual of Omaha sell the most policies, but the same Plan G can cost far less from a smaller company. Use the enrollment numbers to narrow your list, then compare prices in your ZIP code.
