Medicare Supplement plans in Texas are sold by 38 insurance companies on the Texas Department of Insurance list, and Plan G is the overwhelming choice, held by about 60% of Texas’s 995,863 enrollees. Since September 1, 2025, Texas law HB 2516 also guarantees coverage, with price limits, to people under 65 on Medicare because of ESRD or ALS.

Texas is one of the largest Medicare Supplement markets in the country, with close to a million policyholders, and it is one of the most competitive. No single company holds even a quarter of the market, and the state’s seller list runs to 38 insurers.

Texas does not give you a birthday switching window, so the rules on when you can buy matter a lot. This page covers the Texas-specific ones. For what each letter pays, see our Plan G 2027 guide and Plan N 2027 guide.

A Plan G State with No Dominant Company

Medicare Supplement plans Texas 2027: enrollees, Plan G share, number of insurers and top company share

A December 2025 report from Mark Farrah Associates shows how Texans have voted with their premiums. Plan G leads by a wide margin, and the market is spread across several large companies.

Texas figure (Dec. 31, 2025) Enrollees Share
Plan G 602,114 60.5%
Plan F 277,808 27.9%
Plan N 87,946 8.8%
Largest company (UnitedHealthcare) 227,558 22.9%
Second-largest company 203,963 20.5%
Third-largest company 168,444 16.9%

With the top company under 23%, Texas buyers have real choice. That also means the same Plan G can carry very different prices depending on which company you pick.

HB 2516: New Rights for Texans with ESRD or ALS

In 2025 the Texas Legislature passed House Bill 2516. The Texas Department of Insurance explained it to insurers in Commissioner’s Bulletin B-0010-25 on July 22, 2025.

HB 2516 rule Details
Who it covers Texans under 65 who qualify for Medicare because of end-stage renal disease (ESRD) or ALS
Start date September 1, 2025
Coverage Must be offered the same plans as people 65 and older
Plans A, B and D Same rate as a 65-year-old
Other plans, like Plan G Up to twice the age-65 rate
Health screening No medical underwriting, waiting period or pre-existing limit
One-time catch-up window Dec. 1, 2025 to June 1, 2026 for people already on Part B as of June 20, 2025 (now closed)

Under 65 with a Disability: Texas Offers Plan A

For Texans under 65 on Medicare because of a disability other than ESRD or ALS, the rules are narrower. TDI’s guide says you get a six-month open enrollment that starts when you enroll in Part B, but it applies only to Plan A.

Broker tip: Plan A is a starting point, not the end. When you turn 65 you get a fresh six-month window to buy any plan offered in Texas, including Plan G, with no health questions.

Plan A covers Part A coinsurance, Part B coinsurance, blood and hospice coinsurance, but not the Part A deductible or excess charges. Learn how the window at 65 works on our open enrollment period page.

Your Texas Buying Windows, Start to Finish

Medicare Supplement plans Texas 2027 timeline of buying windows from under 65 to age 65 and later

Texas has no birthday rule, so most chances to buy without health questions come at fixed points. Outside them, a company can screen your health and may impose a pre-existing condition wait of up to six months.

1

Under 65 on Part B. Six months to buy Plan A (or any plan if you have ESRD or ALS).

2

Turning 65 on Part B. Six months to buy any plan in Texas with no health questions.

3

Losing other coverage. Federal guaranteed issue rights, such as a Medicare Advantage plan leaving your area, open a 63-day window.

4

Any other time. Apply with health questions; keep your old policy until approved.

Good to know: The fall Medicare open enrollment does not apply to Medicare Supplement plans. TDI’s guide makes this point directly.

Texas Consumer Rules After You Buy

30-day free look. You can return a Texas Medicare Supplement policy within 30 days for a full refund. TDI suggests certified mail with a return receipt.

Equipment charge limit. Texas applies the excess charge limit to Medicare-covered medical equipment, prosthetics and orthotics. Suppliers cannot charge over 115% of Medicare’s approved price without written disclosure and your advance agreement.

Prompt payment. Texas law requires insurers to pay claims promptly. You, your doctor or your hospital can complain to TDI if a claim is delayed.

Plan G covers excess charges in full. Plan N does not, so the equipment rule matters more if you pick N. Compare them on our Plan G vs Plan N page.

Comparing 38 Texas Companies Without a State Rate Chart

Unlike some states, TDI does not publish a premium comparison chart. Its guide points Texans to the Medicare.gov Medigap policy search plus TDI’s list of companies selling in Texas, last updated October 1, 2026. That list notes every company must offer Plan A, and a company selling other plans must also offer Plan C or Plan F.

1

Use the TDI list. See which of the 38 companies sell in Texas, then confirm they sell in your area.

2

Search Medicare.gov by ZIP code. It shows the companies and plan letters available where you live.

3

Call HICAP for free help. Texas’s State Health Insurance Assistance Program is HICAP, the Health Information, Counseling and Advocacy Program.

4

Get several quotes. A licensed agent can quote many companies at once. Our Plan G rate increase guide shows what to ask about future increases.

Bottom Line

Texas gives Medicare Supplement buyers plenty of choice, with 38 companies and no single dominant insurer, and 60% of Texans settle on Plan G. HB 2516 now protects people under 65 with ESRD or ALS. Because there is no birthday rule, buy carefully during your window at 65 and compare widely.
