What Drives Your Medigap Premium

What drives Medicare Supplement plan cost in 2027: age, location, tobacco use, gender and the insurance company

  • Your age. Most states use attained-age pricing, so your premium goes up as you get older.
  • Where you live. Your state and ZIP code have a big effect. Some states cost far more than others for the same plan.
  • Tobacco use. Most states let carriers charge tobacco users more.
  • Gender. In many states, women pay a little less than men for the same plan.
  • The company. Identical plans can cost very different amounts from one carrier to the next in the same ZIP code.

How Medigap Plans Are Priced

Pricing method How it works
Attained-age Based on your current age. Starts lower and rises as you get older. Most common.
Issue-age Based on your age when you buy. Doesn’t rise because of age, but can still increase for other reasons.
Community-rated Everyone pays the same price regardless of age. Can still increase over time.

No matter how a plan is priced, most premiums also go up each year as medical costs rise. Read more about Plan G rate increases in 2027.

Your Total Monthly Medicare Cost

Your Medigap premium is only one piece of your monthly bill. Most people pay:

  • Part B premium: $202.90 a month in 2026 (the 2027 amount hasn’t been announced yet)
  • Medigap premium: depends on your plan, age and ZIP code
  • Part D premium: if you add a drug plan

In exchange, you get predictable costs. With Plan G, the main out-of-pocket cost for Medicare-approved care is the Part B deductible ($283 in 2026).

How to Pay Less for a Medicare Supplement

  • Compare the same plan letter. Benefits are standardized, so a cheaper Plan G is the same coverage for less money.
  • Ask about household discounts. Many carriers, like Mutual of Omaha, offer savings when another adult in your home has coverage.
  • Look at Plan N or High Deductible Plan G. Lower premiums in exchange for small copays or a deductible. See Plan G vs Plan N 2027 and High Deductible Plan G 2027.
  • Buy at the right time. During your Medigap Open Enrollment Period, no carrier can charge you more because of your health.

Medicare Supplement Cost FAQs

➤ How much does a Medicare Supplement plan cost per month?

It depends on the plan, your age, where you live and the company. National averages for a 65-year-old in the 2026 plan year ranged from about $61 a month for High Deductible Plan G to about $220 for Plan G and $271 for Plan F. Your price may be higher or lower.

➤ What is the cheapest Medicare Supplement plan?

High Deductible Plan G and Plan K usually have the lowest premiums. They cost less each month because you pay more of your own costs when you use care.

➤ Why do Medicare Supplement premiums go up?

Premiums can rise because of age, depending on how the plan is priced, and because of yearly rate increases tied to rising medical costs and claims.

➤ Do Medicare Supplement premiums depend on my health?

Not during your Medigap Open Enrollment Period, when carriers can’t charge you more for health reasons. Outside that window, most carriers can use health questions to set your rate or decline you.

➤ Are Medicare Supplement premiums the same everywhere?

No. Premiums vary a lot by state and ZIP code. The same plan letter can also cost very different amounts from different companies in the same area.

➤ What is attained-age pricing?

With attained-age pricing, your premium is based on your current age and goes up as you get older. Most states use this method. Issue-age and community-rated plans don’t rise because of age, though they can still increase for other reasons.

➤ Do I still pay the Part B premium with a Medicare Supplement?

Yes. You pay your Part B premium ($202.90 a month in 2026) plus your Medigap premium, and a Part D premium if you add drug coverage.

➤ How can I lower my Medicare Supplement cost?

Compare the same plan letter across several companies, ask about household discounts, consider Plan N or High Deductible Plan G, and buy during your Medigap Open Enrollment Period.

New to Medicare?

If you are new to Medicare, then you can enroll in a 2027 Medicare Supplement plan up to six months prior to your Medicare Part B effective date.
