Blue Cross Blue Shield Medicare Supplement Plan G 2027: Prices From Four State Rate Guides

Reviewed by Russell Noga, Licensed Medicare Insurance Broker (all 50 states) — Last updated October 29, 2026

Blue Cross Blue Shield Medicare Supplement Plan G is sold by your own state’s Blue company, not by one national BCBS insurer, so the price, discount and rules depend on where you live. In 2026 state rate guides, a 65-year-old man paid $194.32 a month for Plan G from Horizon BCBS of New Jersey, $315 from CareFirst in Maryland, and $477.04 to $497.43 from the Blue plans in upstate New York.

independent Blue Cross and/or Blue Shield companies (BCBS Association)

People type “Blue Cross Blue Shield Plan G” as if it were one product with one price. It is not. The Blue Cross Blue Shield Association says there are 35 independent, locally operated Blue companies. Each one files its own Medicare Supplement rates with its own state, and some sell under names that never say “Blue Cross” on the policy.

So we did the legwork. We pulled the Blue company’s Plan G premium out of official rate guides from New Jersey, Maryland, New York and Nevada and set it next to the other insurers in the same guide. For a company-level look at every Blue plan letter, see our Blue Cross Blue Shield Medicare Supplement plans 2027 review.

Which Blue Company Sells Plan G Where You Live

Blue Cross Blue Shield Medicare Supplement Plan G 2027 prices by state from official rate guides

Your Blue Plan G is issued by the licensed Blue company for your area. A Blue in one state cannot sell you its policy if you live in another state. That is why a neighbor’s BCBS price from across a state line tells you nothing about yours.

New Jersey. Horizon Blue Cross and Blue Shield of New Jersey is the Blue listed in the state’s 2026 Medicare Supplement chart.

Maryland. The Blue product is sold by First Care, Inc., doing business as CareFirst MedPlus. The state guide does not print the words Blue Cross on that line.

Upstate New York. Two Blues share the state: Excellus BlueCross BlueShield, and Highmark (Blue Cross Blue Shield of Western New York and Blue Shield of Northeastern New York).

Nevada. Anthem’s Plan G appears in the state guide under its legal name, Rocky Mountain Hospital and Medical Service, Inc.

Broker tip: When you get a Blue quote, read the company name on the application. That legal name is the one you will see in your state’s rate guide and complaint records.

Blue Plan G Prices Next to Other Insurers, State by State

These are real premiums printed by state agencies. Each guide uses its own age, sex and rating rules, so compare within a row, not across states.

State guide Blue company Blue Plan G Other Plan G prices in the same guide
New Jersey SHIP chart, June 2026 (male 65, preferred) Horizon BCBS of NJ $194.32/mo Low $164.31 (Bankers Life) to high $299.88 (Aetna)
Maryland Insurance Administration, July 1, 2026 (male 65) CareFirst MedPlus, Level 1 $315/mo Low preferred rate $203 (Monitor Life); Medico $217
New York DFS, rates in effect Aug 1, 2026 (community rated) Excellus BCBS $477.04/mo Mutual of Omaha $384.02 to $511.36; Humana $601.17 to $709.98
New York DFS, same table Highmark BCBS of Western NY $497.43/mo Same range as above
Nevada Division of Insurance guide (female 65, Las Vegas, rates surveyed Sept 2024) Rocky Mountain Hospital and Medical Service (Anthem) $3,531/yr Low $1,885/yr (Monitor Life) to high $4,044/yr (Guarantee Trust)

The pattern is clear: a Blue name does not mean the lowest price. In New Jersey, Horizon was mid-pack. In Maryland and Nevada, the Blue rate sat near the top of the range. Our Plan G cost guide for 2027 explains what drives these gaps.

New York: Two Blues, One Price for Every Age

New York requires community rating, so a 65-year-old and an 85-year-old pay the same premium from the same company in the same region. The NY Department of Financial Services lists the Blues side by side.

Upstate NY Blue Plan G (monthly) Plan N (monthly)
Excellus BlueCross BlueShield $477.04 Varies by region
Highmark Blue Cross Blue Shield of Western New York $497.43 $318.24
Highmark Blue Shield of Northeastern New York $492.20 $321.82

NY DFS Comparison of Year 2026 Community Rated Standardized Medicare Supplement Monthly Premiums, in effect Aug 1, 2026. A rate may apply to part or all of a region.

At Highmark Western New York, Plan N cost $179.19 a month less than Plan G in this table, about $2,150 a year. That gap is far bigger than Plan N’s copays for most people. Our Plan G vs Plan N comparison shows how to run the numbers.

Maryland Shows How a Blue Can Price Plan G by Health Level

CareFirst Blue Cross Blue Shield Medicare Supplement Plan G monthly premium by health level in Maryland

CareFirst prices Plan G in levels. The Maryland guide says Level 1 rates apply only during your six-month open enrollment or a guaranteed issue period. Apply later and you go through medical underwriting, which can place you in Level 2 or Level 3.

CareFirst Plan G, male 65 (Baltimore Metro, D.C. Metro, Western MD) Monthly premium
Level 1 (open enrollment or guaranteed issue only) $315
Level 2, non-smoker (medically underwritten) $365
Level 3, non-smoker (medically underwritten) $567
Level 3, smoker $708

Maryland Insurance Administration, Monthly Premiums for Medicare Supplement Insurance Policies, as of July 1, 2026. CareFirst’s household discount is listed at 10%.

Why it matters: The same CareFirst Plan G cost $315 for one 65-year-old man and $567 for another, based only on when and how he applied. Timing your purchase can be worth more than any discount.

Plan G, High-Deductible G or Plan N From a Blue

Whichever Blue you buy from, Plan G pays the same standardized benefits. You pay the Part B deductible ($283 in 2026, projected $292 for 2027) and the plan pays the rest of your Medicare-approved share. The real choice is between Plan G and the cheaper letters your Blue offers.

Plan G. Horizon BCBS NJ charged $194.32 a month in the June 2026 chart. You pay only the Part B deductible.

Plan N. Horizon charged $152.23 for Plan N in the same chart, or $42.09 less each month. You add copays of up to $20 per office visit and up to $50 per ER visit, plus excess charges.

High-Deductible G. Not every Blue sells it. Where offered, you pay the first $3,050 in 2027 before the plan pays. See our High-Deductible Plan G guide.

How to Shop Your Local Blue Against Everyone Else

1

Find your Blue’s legal name. Look at the quote or application. In some states it is Anthem, CareFirst, Highmark, Excellus or Horizon rather than “Blue Cross Blue Shield.”

2

Open your state’s rate guide. Many insurance departments and SHIP offices publish Medicare Supplement premium charts. Find the Blue line and the lowest Plan G line.

3

Ask how the rate is set. Is it community rated, attained age, or tiered by health level like CareFirst? This decides how your bill changes later.

4

Check the household discount. CareFirst lists 10% in Maryland. Other Blues set their own terms.

5

Apply in your protected window. During open enrollment the Blue cannot use your health to charge you more or turn you down.

Watch out: Connecticut’s rate list shows Anthem Blue Cross and Blue Shield rates approved on September 3, 2026 to take effect January 1, 2027. Ask any Blue whether a new rate is already scheduled before you compare.

Bottom Line

There is no single Blue Cross Blue Shield Plan G price. In 2026 state guides, the local Blue ranged from mid-pack in New Jersey to near the top in Maryland and Nevada. Get your local Blue’s Plan G quote, then line it up against the lowest Plan G in your state before you buy.

Blue Cross Blue Shield Plan G — Frequently Asked Questions

➤ Is Blue Cross Blue Shield Plan G the same in every state?

The benefits are the same because Plan G is standardized by federal law. The company, price, discounts and pricing method all change by state, because each area has its own independent Blue company.

➤ Why does my BCBS Plan G quote say Anthem, Highmark or CareFirst?

Several Blue companies sell under their own brand names. Anthem, Highmark, CareFirst, Excellus and Horizon are all licensed Blue Cross and/or Blue Shield companies in the areas they serve.

➤ Is a Blue Cross Plan G cheaper than other companies?

Not by default. In the 2026 guides we checked, Horizon BCBS NJ was in the middle of the pack, while CareFirst in Maryland and Anthem in Nevada were near the top of their states’ ranges.

➤ Can I buy a Blue Plan G from another state’s Blue company?

No. You buy from the Blue licensed where you live. If you move, you may keep your policy, but a new Blue policy would come from the Blue in your new area.

➤ Does my age change a Blue Plan G premium in New York?

No. New York uses community rating, so the Excellus or Highmark premium for your region is the same whether you are 65 or 85.

➤ What is CareFirst’s Level 1 rate?

It is CareFirst’s lowest Plan G tier in Maryland, $315 a month for a 65-year-old man in July 2026. It is only available during open enrollment or a guaranteed issue period.

➤ What is the Plan G deductible with a Blue Cross policy?

It is the Medicare Part B deductible: $283 in 2026, projected at $292 for 2027. After that, a Blue Plan G pays your share of Medicare-approved costs.

➤ Do all Blue companies sell High-Deductible Plan G?

No. Each Blue decides which letters it sells. Ask your local Blue, and compare with other insurers that offer it, since the 2027 deductible is $3,050.

New to Medicare?

If you are new to Medicare, then you can enroll in a 2027 Medicare Supplement plan up to six months prior to your Medicare Part B effective date.
